LSVT BIG Training for Parkinson’s: What It Is and How It Helps

apoyo-persona-con-parkinson

One of the most limiting symptoms of Parkinson’s is the progressive reduction in movement amplitude: steps become shorter, arms swing less, handwriting shrinks, and the voice fades. The body seems to close in on itself. The LSVT BIG programme was created precisely to fight this.

What Is LSVT BIG?

LSVT BIG (Lee Silverman Voice Treatment BIG) is a rehabilitation programme specifically designed for people with Parkinson’s disease. It grew out of the successful LSVT LOUD programme — aimed at voice and speech — and transfers its core principle to movement: working with large, exaggerated amplitude to counteract Parkinson’s natural tendency to reduce the size of all movements.

The programme was developed in the United States and has a strong scientific foundation supported by clinical trials published in journals including Movement Disorders and Parkinsonism & Related Disorders. It is now one of the most internationally recommended interventions for the motor treatment of Parkinson’s disease.

What Does the Programme Involve?

The standard LSVT BIG protocol consists of 16 intensive sessions — 4 sessions per week for 4 weeks — carried out with a certified therapist. Sessions last approximately one hour and are combined with daily home exercises.

Exercises focus on:

  • Large-amplitude whole-body movements: raising arms to maximum height, taking wide steps, fully rotating the trunk.
  • Amplified functional tasks: everyday activities performed with intentionally exaggerated movements to recalibrate self-perception of movement.
  • Gait training: longer steps, greater arm swing, improved posture.
  • Balance and transfer exercises (sit-to-stand).

Why Does LSVT BIG Work?

People with Parkinson’s have difficulty internally perceiving the size of their own movements. What feels like a ‘normal’ movement to them is actually much smaller than it should be. LSVT BIG works on sensorimotor recalibration: training the brain to associate ‘normal’ with large movements, until that amplitude becomes automatic.

Studies show significant improvements in walking speed, step length, balance, quality of life and objective measures of movement amplitude, maintained even months after completing the programme.

Who Is LSVT BIG Suitable For?

LSVT BIG is indicated in the early and middle stages of Parkinson’s disease. The sooner it is started, the greater the benefits, because the programme takes advantage of existing neuroplasticity and establishes a broad foundation of movement before reduction progresses further.

It is not exclusive to idiopathic Parkinson’s: it is also applied in atypical parkinsonisms and other conditions associated with reduced movement amplitude.

LSVT BIG at Rehabot

At Rehabot we apply the principles of LSVT BIG integrated within personalised rehabilitation programmes for each patient with Parkinson’s. We offer both the original intensive format and adapted home-based modalities, so that distance or reduced mobility are never a barrier to receiving the best available rehabilitation.
